The doctors welcome new patients who live within our practice area. In order to register at this practice you will need to complete the New Patient Registration Forms for each person wishing to register and return it to the surgery either by hand, by email or by post. You’ll find the contact details on the front page of the form. We would ask you to provide two separate forms of identification, one of these must be photographic, for example a passport or your driving licence and a utility bill with your address on not older than 3 months.
All patients will have a named doctor allocated who is responsible for coordinating your care. You are able to continue to talk to or see any GP of your choice. It doesn’t have to be your allocated GP.
