Access to medical records policy July 2018
The computer records held by the practice are used in the strictest confidence and in accordance with the Data Protection Act. If you wish to opt-out of national data extractions or Summary Care record sharing please complete the opt-out forms available at reception.
Patients may apply to view their medical records under the Access to Health Records Data Protection Act 1998 (DoH June 2003). Requests must be in writing and a fee is payable. Full details and request forms are available by request from the surgery.
