How to register with us
Denton Village Surgery welcomes new patients who live within our catchment area. Please check if you fall within our catchment area by searching your postcode at the following link:

Please note all applications will be verified. If you unfortunately fall outside of our designated catchment area, we will be unable to register you. Once you have checked that your address is eligible, please complete the New Patient Registration Form online using the link below.

If you are unable to complete the form online, a paper version is available. You can collect it from the surgery or download it, then return it in person along with:
- Two forms of identification:
- One must be photographic (e.g., passport or driving licence)
- The second must show your current address, such as a utility bill dated within the last 3 months
Registering Children
Children under 16 must be registered by a parent or legal guardian.
- A separate registration form must be completed for each child.
- You must provide the child’s full birth certificate to verify your relationship.
- In addition, please bring two forms of identification for the parent or guardian (as outlined above).
Your Named GP
All registered patients will be assigned a named doctor responsible for coordinating your care.
However, you are always welcome to see or speak with any GP at the surgery — it doesn’t have to be your named doctor, all clinicians will have access to your full medical history.
Consent to Share Information and Carers
In a GP surgery, explicit consent is essential because patients have a legal and ethical right to control how their personal information is used. Medical records contain highly sensitive details, and sharing them without clear permission could undermine trust and breach confidentiality laws such as the UK GDPR. By asking for explicit consent, the surgery ensures that patients understand what information will be shared, with whom, and for what purpose, protecting their privacy and maintaining confidence in the care they receive.

Change of personal details
If you change name, address or telephone number, please let our receptionists know by calling or writing to us. Alternatively you can use

If you move outside the practice area you may need to find a doctor in your new area.
