Social Prescribers

Who are the Social Prescribers?

A Social Prescriber is a health professional who works with your GP Practice to help you improve your health and wellbeing.  By working closely with your GP and other healthcare professionals, they can provide you with a helping hand to achieve your health and social needs.  They can also help access a wide range of support services provided by the NHS, Local Authority, and voluntary/social sectors.  They will give you time to talk to discuss your worries, fears, and concerns.

Many things in everyday life can affect your health and wellbeing and get in the way of making the changes you need, Social Prescribers are here to help.

 

How can a Social Prescriber help me?

  • Support to make positive changes
  • Give you time to express your feelings, thoughts, and concerns
  • Signposting/help you to access appropriate services to support you with benefits advice, financial problems, bereavements, and substance misuse
  • Advice on employment, training, and education
  • Help with exercise, sports, fitness classes etc

 

Working with a Social Prescriber

  • Together, discuss the problems, issues, and concerns you are facing
  • Devise a social prescription of actions
  • Make goals and aims you can work towards
  • Help you find and access local support networks and agencies
  • Make you feel supported, listened to, and valued
